Full Time Outreach Coordinator

Onward Transitions is a privately owned and operated short term non-residential living service that supports emerging adults (members) ages 18-30, living independently in their own apartments in Portland, Maine. Members are bright, motivated, and supported by their families in their pursuit to gain autonomy and live independently. Member challenges including mild to moderate anxiety, depression, and/or executive functioning challenges. We are not a recovery program, nor do we provide acute care. From a family styled home on the border of USM-the Pine House- and an office suite in the Arts District of Portland, we practice generalized living skills and provide meals, a safe community, therapy and social, career, vocational and life coaching, apartment consultation, social activities, and community outreach. Our members are referred to us by educational consultants and mental health professionals from across the country. They work with us for an average of 9-12 months.

Onward Transitions is currently hiring one full-time staff member to join our team in the newly created role of Outreach Coordinator. The role focuses on helping members achieve their goals of greater integration into the community of Portland. Positive energy, a sense of humor, and flexibility are assets in this role.

The ideal candidate will have the ability to work as part of a team and independently, have a background in residential mental health, experience with recreation service delivery and a knowledge of employment, community service and social recreational opportunities of the Greater Portland Area.

Essential Functions:

  • Serve as a positive mentor to our members
  • Work closely with a team of therapists, coaches, and staff and participate in monthly supervision
  • Meet individually with members as needed (weekly) for coaching around treatment goals as defined in their individualized Family Agreement
  • Develop comprehensive outreach program with Program Director and Clinical Director
  • Design curriculum and lead twice weekly life coaching groups in assigned topics geared towards work and service in Portland
  • Maintain coaching group resources for assigned topics around our Family Agreement
  • Plan and lead once monthly group activities and service projects
  • Manage work site visits as directed by program coordinator (minimum every other week)
  • Cultivate relationships with employment and community service resources
  • Maintain daily milieu notes for members
  • Assist with daily operations in the Pine House and Hay Loft office space (coverage, meals, cleaning, yard work)

Requirements:

  • Must be 25 years or older
  • Must possess a combination of related education and work experience
  • Must submit to a criminal records and driving history check, have a valid license and insurance
  • Most have 3 professional written letters of reference
  • Must maintain appropriate professional boundaries with members

Preferences:

  • Experience working with young adult/adolescent populations in educational or mental health environments.
  • Knowledge of Portland resources and recreation opportunities
  • Possess a breadth and depth of talents and interests, appropriately shareable with our member community
  • CPR/SFA current certification
  • Tech/G-Suite/Social Media savvy

Part Time Logistical Support Staff

Onward Transitions is currently hiring one part-time staff member to join our team in the newly this newly created role. The role focuses on helping to support ongoing service operations in the program. Positive energy, a sense of humor, and flexibility are assets in this role.

The ideal candidate will have the ability to work as part of a team and independently, have a background in mental health or healthcare, experience within the hospitality and service industry (ideally the food service industry) and have a solid driving record. Must be willing to work Tuesdays Wednesdays and Thursdays, with some flexibility around hours.

Essential Functions:

  • Serve as a positive mentor to our members while maintaining professional boundaries
  • Work closely with a team of therapists, coaches, and staff
  • Assist with operations at our Pine House location (shopping, shuttling members, prepping and preparing meals, cleaning, light yard work)
  • Have the ability to redirect members questions and concerns to the appropriate staff

Requirements:

  • Must be 25 years or older
  • Must possess a combination of related education and work experience
  • Must submit to a criminal records and driving history check, have a valid license and insurance (CDL a plus)
  • Most have 3 professional written letters of reference
  • Must maintain appropriate professional boundaries with members

Preferences:

  • Experience working with young adult/adolescent populations in educational or mental health environments.
  • Possess a breadth and depth of talents and interests, appropriately shareable with our member community
  • Detail oriented
